We pride ourselves on supporting people to live the lives they choose in the communities that they call home. Our Community Outreach Support Team offers an objective-led and flexible service that is tailored to a person’s needs and aspirations. We work with individuals and small groups to facilitate access to opportunities where they live.

This exciting service is available in the East Midlands (Nottingham, Nottinghamshire and Derbyshire) and the Southwest.

We provide person centred day services operating out of four sites within Nottingham City and Nottinghamshire. Each site operates as a regional hub for staff and clients to operate from with a mix of activities taking place in the community and on site. We specialise in small group working which enables us to stay committed to our person centred approach.

Our complex needs support works by placing the person at the centre of their lives. This is a smaller and more intensively staffed service which has a therapeutic approach and aims to broaden people’s experiences.

We have a new working partnership with Nottingham City Council providing support for people in their homes. This addition to our provision is an exciting opportunity which enables us to provide the same high level of support and care to people both in the community and in their living space.
